Plenary session II Reshaping services, building partnerships, improving outcomes Well-being is influenced by a range of factors including health, social, employment, culture, housing and the environment. Current reforms and adaptations relate to the impact of the economic crisis and the need to become more efficient and effective. However, the influence of demographic change is ever-present and the desire for a more person-centred approach is growing stronger among citizens. How might services work with individuals, families and communities and across disciplines and sectors to transform lives for the better and reduce health and social inequalities? In the morning session, we pay particular attention to health inequality, housing and homelessness and community development and in the afternoon focus on mental wellbeing and preventative strategies for older people and how new technologies are helping transform lives through social inclusion.
